A high proportion of prisoners come from socially-excluded sections of the community, many have mental health or substance abuse problems, and morbidity rates for all diseases and conditions are higher than normal. Prisoners access health care far more often than patients in the community, perhaps due to enhanced levels of stress and lack of a support network. Gross dental caries in prisons are more than ten times higher than normal, and a significant proportion of patients present complex multiple pathology due to old age.
