All I want...
..is integrated primary and secondary care
All you want is SystmOne
SystmOne is a complete clinical and administrative system for use across all departments of a hospital, creating a seamless patient journey from the moment the patient is referred to hospital or contacts A&E, to the moment their care is passed back to the GP.
But it is also much more than that. Where a patient is cared for by multiple teams who all use SystmOne, their full patient record is available at each and every point of care. Information availability is key to good clinical care, and the integration of primary and secondary care information has the potential to transform patient experiences in the UK.
"Integrated care is the holy
grail. Using SystmOne, we have
an opportunity to transform the
way care is provided."
The government's vision for the future of health care in Britain involves setting up new points of care, with complete and up-to-date patient records available at each and every one, at the very moment they are needed.
SystmOne is the first system to deliver fully integrated care across all care settings. Where patients have been treated at other organisations using SystmOne in the past (e.g. at GP/Community/Palliative/Out of Hours/A&E units), the details recorded are available to clinicians at your hospital, ensuring that decisions are supported by the full knowledge base about that person.
Couple this with electronic hospital discharge letters sent directly to GPs, and future integrations that will enable the information captured by paramedics in ambulances to be filed to the full patient record, and the integrated electronic record becomes a rich and invaluable resource for patient care.
SystmOne also integrates with Spine to provide the Summary Care Record, Patient Demographic Service, Electronic Transmission of Prescriptions, and fully integrated Choose & Book.
And how realistic is this idea of integrated care?
This isn't a pipe dream. It's happening now.
The SystmOne database currently holds over 14 million patient records, that's more than one in four of England's population. If you treat one of these 14 million patients, their patient record will be available to you.
Almost 1,100 GP practices across England use SystmOne - that's more than one in eight practices.
SystmOne is used in Child Health organisations in 42 PCTs, and Community units in 54 PCTs.
Out of Hours services in 22 PCTs use SystmOne, including all urgent care services in the county of West Yorkshire.
Six Community Hospitals are live with SystmOne, with two large hospitals due to go live early in 2010.
The figures speak for themselves, but the more organisations that start to use SystmOne, the more information can be made available at the point of care, and the more your patients will benefit.
Don't just take our word for it...
"It made sense to look into
using SystmOne in the
Community Hospital setting
because the Community
Nursing teams in our area
use SystmOne and any opportunity
to have data shared across
services benefits patient care."
Lee Rickles, NHS East Riding of
Yorkshire, prior to the first go
lives of SystmOne in a hospital setting
earlier this year
Community Matron Maggie Bean manages
141 patients in the community hospital
and home settings with SystmOne.
She says,
"SystmOne has made a
huge difference to managing
a large and complex caseload
of patients. I know immediately
if the urgent care team has
visited a patient overnight
and what to expect when
I visit. Integration with the
therapy and rehabilitation
services is a very
positive
development which will help
us to really streamline the
care offered to patients
in Bridlington."
